Surgical Aspects of Gastric Fundus Ulcer of Bleeding, Perforation and Malignancy

Abstract

|
Among the gastric ulcers of peptic ulcer diathesis, juxta-esophageal area including fundus ulcers of bleeding, perforation and malignancy were presented through the individual case report, and contemplated to have their academical significance especially in the aspect of surgical technique.
That is, bleeding from fundus ulcer was managed by the classical Kelling-Madlener operation with suture ligation of bleeding vessels, exposed on the ulcer bed, and perforation of fundus ulcer was managed by the simple closure with the general measurement for panperitonitis, and malignant ulcer of fundus was managed by the extended total gastrectomy instead of preoperatively planned proximal gastrectomy, because of the submucosal spread of tumor cell into the pyloric antrum.
